NetTeller Enrollment Authorization

I certify that the information provided in this enrollment form is accurate. I authorize Monticello Banking Company (MBC) to activate my accounts for Internet Banking access. I understand my account numbers are set up as a related group and that I can transfer to/from all numbers in that grouping as capabilities and/or restrictions allow. To access Internet Banking functions I understand that I will need access to a computer that satisfies the minimum web browser requirements for online banking, which is 128-bit encryption.

ONLINE BILL PAYMENT: By using Online Bill Payment I authorize Monticello Banking Company to post the payment transactions generated from the Online Bill Payment service to the account indicated on my NetTeller enrollment form. If at any time I decide to discontinue service I will provide notification to Monticello Banking Company. I understand that payments may take up to 10 business days to reach the vendor and that they will be sent either electronically or by check. Monticello Banking Company is not liable for any service fees or late charges levied against me. I also understand that I am responsible for any loss or penalty that I may incur due to lack of sufficient funds or other conditions that may prevent the withdrawal of funds from my account. Online Bill Payment is a free service, however If your online Bill Payment account is inactive for a minimum of two months then MBC may deactivate your Online Bill Payment. Fees are subject to change at MBC's discretion.

NETTELLER ACCESS AGREEMENT: The Monticello Banking Company's NetTeller Access Agreement ("NAA") for accessing your accounts with NetTeller, explains the terms and conditions governing the following services: account access ("NetTeller"), bill payment ("Bill Pay"), and other services offered through Monticello Banking Company Online. These services and features will be collectively referred to as "Online Services". The accounts that may be accessed with NetTeller include accounts maintained with Monticello Banking Company or its affiliates ("MBC"). The accounts that you are given access to through NetTeller will be referred to as "the accounts". This NAA will be governed by and interpreted in accordance with all applicable federal laws and regulations. The following rules will apply to the extent there is no applicable federal law or regulation. The account or service you are accessing via NetTeller is maintained through Monticello Bankshares, Inc. and will be governed by and interpreted in accordance with the laws of the State of Kentucky. The terms "we," "us," "our," and "Bank" refer to Monticello Bankshares, Inc. "You" refers to each signer on an account. The term "business day" shall mean every day except Saturdays, Sundays and federal banking holidays. MBC may, at times, introduce new online services or features. This NAA will be updated to inform you of the existence and conditions of these changes. NetTeller Access Agreement
